Deformity refers to someone who was born with a biological defect. This can be something mundane such as having an extra toe, or more severe such as having a hunchback or a second butt-hole. For purposes of this database, deformity is distinguished from disfigurement, which describes people who were born otherwise normal, but from some sort of physical abnormality due to external causes.

- Friday the 13th: Alice Hardy has a dream in which she sees young Jason Voorhees attacking her. He has severe facial deformities including a lazy eye, a disjointed jaw and a lumpy head. This representation is repeated with the first look of the adult Jason in the second film and remains a constant throughout the series as well as the 2009 remake. After the fourth movie however, Jason becomes a supernatural creature and his deformity is no longer considered a "natural" condition.
- Friday the 13th Part 2: Jason Voorhees' freak-ugly face is shown as an adult for the first time. He is seen unmasked at the very end of the movie when he leaps through the window to attack Ginny and Paul.
- Richard III: Based on the life of the actual Richard III of England, it presents King Richard with a crooked posture, severe Kyphosis (hunchback) and a lame arm.
- Tower of London (1939): Based on the life of the actual Richard III of England, it presents King Richard with a crooked posture, severe Kyphosis (hunchback) and a lame arm.
